Change of Personal Information

This form allows current Oblate School of Theology students to officially update their personal information on file with the Office of the Registrar. Students can request changes to their legal name, mailing address, phone number, date of birth, or Social Security number by completing the appropriate sections of the form and providing required documentation for verification. All requests must include the student's electronic signature and supporting documents such as a Marriage License, Social Security Card, Birth Certificate, Passport, Court Order, or Driver's License, depending on the type of change being requested. Students may submit the completed form along with documentation in person, by US mail, email, or fax to the Registrar's office for processing.
