OST Health Insurance Waiver

This form allows eligible students to waive enrollment in the OST Consolidated Health Plan by submitting proof of qualifying health insurance coverage. All students must complete and submit the waiver by the published deadline each semester to avoid automatic enrollment and associated charges.
